Please fill out the Client Intake Form at least 4 hours in advance of your scheduled appointment time. Otherwise, you must arrive 15 minutes early to fill out the paper version at the office.

Cancellation Policy:

If you are  unable to make a scheduled appointment or have an illness, you agree to cancel or reschedule the appointment at least 24 hours in advance. You agree to pay 50% of the full session rate if you give less than 24 hours’ notice.

You agree to pay FULL session rate if you give 4 hours' notice or less, or if you miss an appointment without giving any notice (NO SHOW).

Arriving Late:

Out of respect and consideration of my time and other clients, please plan to be on time. Arrive 10 minutes early for any appointment in order to get the full time scheduled. If you arrive right on time or late, I can only give you whatever time remains of your appointment and you will be responsible to pay the full length of the session you requested.

*This time is scheduled just for you and only you which is why I require a credit card on file or prepayment to reserve and guarantee your appointment. However, it WILL NOT be charged unless you do not adhere to the Cancellation/No- Show policy.

If you do not wish to save a credit card number on file, no problem! You can prepay your service online on my website. Cancellation/No-Show policy will still apply.
